What is Tendonitis?
Tendonitis is inflammation of a tendon. Tendonitis can occur anywhere in the body where we have tendons, and commonly occur in the shoulder, elbow, wrist, hip, knee and ankle joints. Over the years, tendonitis had become a blanket term thrown about for all conditions affecting the tendon, as very often these conditions included some degree of inflammation. However inflammation is not always present, and therefore the vocabulary has been expanded in recent years to more accurately reflect different conditions. For example, the words tendinosis, or tendinopathy, are now more commonly used to describe conditions where the tendon has become damaged or has lost function.
Tendonitis itself (ie. the inflammation) can also arise from more systemic causes, such as infection, however tendinosis usually occurs from overuse, sudden excessive load, or trauma. Symptoms can include pain, especially on carrying out certain tasks and movements, swelling, restricted range of movement and a warm/hot feeling on the skin around the affected area.

Simple active range of movements (ie. movements performed by the client), resisted range of movements (client resisting pressure from the client), and passive range of movements, (movements of the client’s affected joint carried out by the therapist), can quickly help the therapist identify if there is dysfunction in the tendon. Additionally, any sign of heat, redness, swelling and pain on applying pressure to the affected area indicate inflammation, which is suggestive of tendonitis.
Our massage techniques to reduce inflammation involve the use of muscle energy techniques. In particular, the contract-relax technique, performed several times for each affected neuromuscular tendon unit, help to bring nutrition into the affected area, and remove waste products, thereby helping to bring the inflammation down. Massage strokes are performed gently and in a specific direction to ensure that the tendons are not further inflamed and that any new fibres laid down by the body are not disrupted.

The key to quick recovery from tendonitis, in addition to orthopaedic massage therapy, is a programme of stretching and strengthening carefully tailored around the individual client. Neither stretching nor strengthening can be aggressive in the early stages, as the risk of reinjuring the tendons is high.
Certain, specialised exercises, known as eccentric exercises are usually better to speed up recovery, but only when the exercise is pain-free for the client. It is best to commence with a resistance type of exercise initially until the affected tendons can take on stronger loads and repetitions, and be completely pain free for the client.
Eventually the client is progressed onto concentric range of movement exercises, under increasing load until the injured tissues are fully rehabilitated.
