Anterior hip pain due to osteoarthritis is usually a deep achy pain, which is worse on waking up, gets better on “warming up”, then gets worse during the day. A physician using x-ray technology can easily diagnose it. There are also a number of tests which can be carried out in clinic which could indicate hip osteoarthritis.

Groin strains are a common cause of anterior hip pain. Muscles commonly involved are the iliopsoas muscle/tendon, which is a powerful hip flexor, and the adductor longus on the inside of the leg. These strains usually occur from sport or accidents.

There are also little fat pads called bursas, underneath muscles in the groin area. They help muscles to glide over other structures. Sometimes these bursas can get inflamed or dry out due to improper mechanics, leading to bursitis, which can also be painful.

Another condition known as anterior femoral glide syndrome (AFGS) can cause a lot of pain in the groin on lifting the leg (hip flexion), because the femur (thigh bone) does not get anchored properly in its socket on flexing the hip. This condition arises from muscle imbalances such as short flexors and weak gluteal muscles (buttocks).

There is also a condition which causes a snapping sound called Snapping Hip Syndrome.
